Terms and Conditions
Do you agree to the following terms and conditions?
Payment and Refund Information
1. Payment for the class is due upon registration.
2. If a student drops from the program prior to the start of the first class, a full refund will be provided.
3. For new students. If a student new to the program drops from the program after the first class, a full refund will be provided as long as Global Minds, LLC is notified by email at least 24 hours before the start of the second class. If a new student drops from the program within the first three classes, a pro-rated refund will be made as long as Global Minds, LLC is notified by email at least 24 hours before the start of the fourth class. No refunds will be provided outside of this window.
4. For students who have participated in at least one session in the past six (6) months. Returning students dropping after the first class will receive an 50% refund as long as Global Minds, LLC is notified by email at least 24 hours before the start of the second class. No refunds will be provided outside of this window.
5. If a class is canceled for reasons outside of Global Minds, LLC’s control (e.i. inclement weather, building is inaccessible, act of god, etc.), classes will be rescheduled to the next available date. If a class is cancelled for other reasons, refunds or credits will be issued for the pro-rated cost of the cancelled class.
Attendance Rules
1. Students who will be absent from a class meeting must notify the instructor by email prior or within 24 hours after the class.
2. Students who are repetitively absent from class meetings without notification can be dismissed from the program. No refund will be issued.
3. Students will be released from class only at the end of a class meeting.
4. When students miss class, they are responsible for reviewing missed materials which will be posted online.
Code of Conduct
A student can be dismissed from the club for reasons including, but not limited to: violating Federal/State laws, breaking team Rules and Regulations, accruing an excessive number of class absences, participating in disruptive behavior during class and/or conference, consistently not doing classwork/homework. No refund will be issued.
Conference Registration Regulations
1. Students who register to attend local Model UN conferences are expected to arrange their own transportation to and from the conference.
2. Students who register for a local conference and then cancel will forfeit any refunds.
Media Release
I acknowledge that students may be photographed and videotaped during meetings and conference events and those photographs and video may be used in various Global Minds, LLC publications. I hereby grant Global Minds, LLC permission to use my child’s likeness in a photograph or video in any and all of its publications, including websites or other channels of media, for the purposes of publicizing Global Minds, LLC programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product wherein my child’s likeness appears.
Coronavirus/COVID- 19 Regulations
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Global Minds, LLC can not guarantee that my child/ren will not become infected with the Coronavirus/Covid-19. I voluntarily seek services provided by Global Minds, LLC and acknowledge that there is a risk to exposure to the Coronavirus/COVID-19. I acknowledge that my child/ren must comply with all set procedures to reduce the spread while attending classes, such as wearing a mask over their nose and mouth, and frequently washing hands.
I attest that:
* I will not send my child to class if they are experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* My child has not traveled to a highly impacted area within the United States of America in the last 10 days.
* I do not believe my child/ren has been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold Global Minds, LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself, my child/ren and/or property that may be caused by any act, or failure to act by Global Minds, LLC. I understand that this release and waiver discharges Global Minds, LLC from any liability or claim that I, my heirs, or any personal representatives may have against Global Minds, LLC with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Global Minds, LLC. This liability release and waiver extends to all owners, partners, and employees of Global Minds, LLC.
Authorization
I hereby certify that I am the parent or guardian of the child for whom this registration is for.
